Getting started
New Hope can answer questions and discuss availability. Service selection and authorization remain part of the DDS person-centered planning process.
Discuss the individual’s goals, needs, authorized services, and interest in New Hope.
The individual, guardian, or case manager can contact us to discuss fit and current capacity.
If New Hope is selected and authorized, we coordinate with the support team for a safe, person-centered start.
