Choosing a provider is a person-centered decision. New Hope welcomes conversations with individuals, guardians, families, and DDS case managers who want to learn about our services and current availability.
For individuals, families, and guardians
Talk with the individual’s DDS case manager about authorized services and provider options. Then contact New Hope with the service being considered, general area, preferred schedule, and the best person for follow-up.
For DDS case managers and referral partners
New Hope currently welcomes HTS referrals as well as DLS and transportation availability conversations. Share only the minimum general information needed for an initial discussion. We will arrange a secure method if protected records are required.

Talk with the DDS case manager
Review the individual’s goals, assessed needs, authorized services, Plan of Care, and provider options. The case manager can explain eligibility and official next steps.

Contact New Hope
Tell us which service is being considered, the general support needs, preferred location or schedule, and who should participate in follow-up. Please do not email highly sensitive records through the general contact form.

Discuss fit and capacity
We will discuss our service approach, current capacity, staffing considerations, and whether more information is needed through an approved secure channel.

Complete provider selection and authorization
If New Hope is selected, the case manager and planning team complete the required provider and authorization steps. New Hope cannot begin before the necessary approvals are in place.

Coordinate a person-centered start
We work with the individual and authorized team members on introductions, routines, risks, communication preferences, documentation, and a thoughtful service transition.
What to share in the first inquiry
- Your name and role
- HTS, Daily Living Supports, or transportation
- City or general service area
- Preferred days or general schedule
- Whether a DDS case manager is involved
- Preferred method and time for follow-up
Protect personal information: Do not submit diagnoses, Social Security numbers, Medicaid or member IDs, medical records, Plans of Care, medication information, incident records, a DLS home address, or other sensitive details through the general form.
